    CA STATE DISABILITY

    STATE DISABILITY INSTRUCTIONS:

     

    • Visit http://www.edd.ca.gov/

    1. for more information and tutorials if you need.
    2. Disability forms can be filled out either online (the easy way), or you can request the form to be sent to you to be filled out by you then sent to us for completion and we will send the form in for you.

    • The SDI Online process can be started by first registering by clicking 

     

    • How To File Link.
    • Once the registration is complete, you may log in and complete your claim by visiting the SDI Online Login page.
    • Complete all of the information that you are allowed to enter, you can “pre-enter” all of this before you are off work to get your claim started, but the website will not allow a future date to be entered so your claim cannot be submitted until the day after your last day of work.


    Disability Evaluations and from requiting Policy


    At Khiem Mai Medical, our focus is on comprehensive primary care, preventive medicine, and metabolic health optimization.

    Disability evaluations and determinations often require specialized assessments that fall outside the scope of routine primary care. For this reason, our practice does not complete disability forms unless there is sufficient medical documentation and clinical evidence to support the request.


    Note: Completion of disability or other medical forms is based on the physician’s clinical judgment and review of the patient’s medical condition and records.

    • Not all applications will be approved or signed by the physician.
    • Submission of a form does not guarantee approval of benefits by any agency or insurance provider, including the California Employment Development Department.
    • The physician’s role is to provide accurate medical information only; all final determinations are made by the reviewing agency or insurer.
    • Administrative fees may apply for completion of forms and medical documentation.

    Administration fee for EDD:

     WE REQUIRE A $50 PAYMENT WHEN ANY EDD FORMS REQUIRING COMPLETION ARE DROPPED OFF. FOR ALL OTHER FORMS,  LIFE INSURANCE FORMS,  AFLAC, ETC, WE REQUIRE A $30 PAYMENT.  THERE WILL BE A 7-10 BUSINESS DAY TURNAROUND FOR THIS SERVICE.  IF YOU WOULD LIKE YOUR PAPER WORK EXPEDITED (24-48 HOUR TURNAROUND), THERE WILL BE AN ADDITIONAL $10 PROCESSING FEE.

    The only documentation regarding your health or illness required by law (and included in the office visit charge) is an office visit note.

    Completing paperwork for schools, camps, the Family Medical Leave Act (FMLA) claims, long-term care, life insurance, the Department of Veterans’ Affairs, disability claims or other purposes is unnecessary duplication and goes beyond routine medical care. Therefore, it cannot be billed to your insurance company. Since all forms require our signature, we are personally responsible for the accuracy of the information provided. Incomplete or inaccurate information may have far reaching consequences for your case. Filling out forms thus requires careful consideration and considerable amount of our time.

    Therefore, it is our office policy to charge for the completion of any form as follows:

    – Processing fee of $30 per form, and

    – Completion fee of $5 per each page.

    – EDD fill out Administrative fee 

    $30 → Online EDD submission (recommended)

    $50 → Paper form 

    We cap the charge at $50 maximum per form. We will complete the form and fax it to the designated recipient (or return it to you if you prefer) within 3- 7 business days of the receipt of payment.

    * (except $75–$100 → Rush / same-day complete)  

    Please email the specific form you would like us to complete to jmai@khiemmaimedical.com  Once received, we will send you an invoice of the amount to complete the form. To avoid delays, please make the payment once invoice is received.

    If the form to be completed was sent to us by an organization, we will notify you of the exact amount that is due. We may also request completion of the Authorization to Release Medical Records. The form will be completed and sent to you (or the requester) within 3 business days after the payment is received.

    You can avoid being charged for the form completion by requesting that the requester accepts a copy of the office visit note in lieu of a form. (For example, there is no requirement to use the FMLA form by employers. The employer just needs to know that the medical condition is real and qualifies for FMLA leave.) We can send to the designated recipient a copy of the last office note free of charge. If you wish us to do so, please email us a signed Authorization to Release Medical Records  to jmai@khiemmaimedical.com

    Finally, completion of certain forms, such as school forms, camp forms, sport participation forms, disability determination, etc., may require an update of your medical information or a special examination. In such cases, you will be asked to make an appointment and we will fill out the form as part of the office visit and we will not charge you for completing form.
